Various changes:

* Add `pasink list --name` to list just the sink names. Useful for shell completion.
* The displayed volume should now be the average of all channels (instead of just the first two).
* Decode pacmd output with errors='replace'. Probably only matters for stream names, which we don't use.
* Better comments and minor code shuffling.
