Insert values for the union of all pairs at every source location

Previously, we did not insert values at source locations where:
1. The source did not have an exact entry for the kerning pair; and/or
2. A class-class pair had a value of 0.

As OpenType's variation model treats the absence of pairs differently
to the DS+UFO kerning model, this meant that such pairs respectively:
1. Had interpolated values where they should have fallen back to the
   value of more general kerning entries; and/or
2. Had interpolated values where they should have had a value of 0.

This commit corrects this, reducing diffs between the merge and
variable-first feature compilation approaches, although still needs
testing to understand the scope of any remaining diffs, and tidying
before the implementation is as clean as it can be.
