Change lexical_cast<std::string> to std::to_string

I didn’t do it on every instance of the former, just on the ones directly involved with fixing #256 and also a few where probably the behavior of that bug would have been replicated.

For reference: It’s boost::lexical_cast() that was adding the thousands separators.
