Update requirements.txt for setuptools

Trusty venvs have setuptools 2.2, whereas Xenial venvs have
setuptools 25.2.0. This has begun to cause issues, such as
in https://github.com/juju/charm-tools/issues/246.

This change pins the setuptools version to 25.2.0 to both
address the issues that have appeared in charm-tools reqs,
and to ensure the same setuptools version across Trusty and
Xenial virtualenvs.

Ensure required interfaces list is correct

When a subset of cinder services is enabled, we don't expect all interfaces
to exist and unexpected errors/warnings to be reported in workload-status.

Use bundletester for amulet test execution

Switch to using bundletester for execution of functional tests,
leveraging tox to build out test virtualenvs.

Rename amulet tests inline with gate-*, dev-* and dfs-*
naming standards.

Update README to refer to functional testing section of the charm
guide.

Install python dependencies early for CH ip.py

When using charmhelpers.contrib.network.ip apt install was quietly
failing. Install python dependencies in the early install hook
(before install.real).

Charm-helpers sync to bring in apt_install with fatal=True

Various deploy from source changes and c-h sync

* openstack-origin is no longer required when using openstack-origin-git.
* Drop kilo from openstack-origin-git default values because upper
  constraints doesn't work in kilo for openstack-dashboard and glance.
* Add flag to allow for disabling upper constraints when using
  openstack-origin-git with user-specified git repositories.

Resync charmhelpers

Due to the way that changes get merged in git/gerrit, a number of
charm-helpers changes from a previous sync where lost when an
older review was landed, which referred to an older charmhelpers
version.

Resync and fix missing bits.
