Start adding the framework for Mozmill tests, in preparation for some heavy refactoring.

Improved the script to run tests, so that it now deleted the test database when it finishes. Updated the README accordingly.

Clean up the NOINIT stuff, which we don't yet use; make sure we run a separate Thunderbird process; make tests use a dedicated CouchDB database.

Started adding the infrastructure to be able to run tests, along with a very simple test file with one passing and one failing test. Updated the README with details, as long as updating the instructions for debugging.
